General Summary:
- Works under the supervision of the hospice manager and Vice President.
- In the absence of the hospice Manager would assume those responsibilities as assigned.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Assesses and identifies patient needs in accordance with the hospice concept of care.
- Coordinates with the Interdisciplinary Team to provide complete patient/family care.
- Maintains the continuity of care by ensuring that patient needs are met.
- Nursing equipment, laptop, supplies and forms necessary to provide quality patient care.
- Use of protective equipment as needed for standard precautions.
- Completes home visits, providing accurate and timely assessments of the family/patient enabling identification of needs.
- Formulates and implements the individual plan of care with the medical director and Interdisciplinary Team based on the patient/family assessment and hospice standards of care.
- Manages discomfort and provides symptom relief.
- Evaluates the effectiveness of the interventions through regular monitoring, observations and reassessment of the patient/family situation.
- Makes revisions as needed.
- Monitors the quality of care provided for the patient through participation in quarterly monitors according to the Quality Assurance Plan.
- Participates in the orientation of employees in relation to the function of hospice nurse.
- Displays sensitivity in discussing the reality of impending death and hospice care with patients, families, physicians and community groups.
- Documentation in the patient medical record is concise, timely, objective information which impacts patient care.
- Assumes the duties of the hospice manager in his/her absence as assigned.
- Monitors the patient care activity of the home health aide and the family's private care giver; registered nurses, licensed practical nurses and sitters.
- Reports discrepancies to the hospice manager immediately. Involves the family in the plan of care, providing the family with insight into the patient's needs while concurrently understanding the family's needs.
- Prioritizes the needs of the patient/family and meets those needs in a timely fashion.
- Displays judgment and decision making skills by having patient/family needs met to prevent unnecessary use of on-call service for non emergent problems.
EDUCATION AND EXPERIENCE:
- Graduation from an accredited school of nursing.
CERTIFICATIONS AND LICENSURES:
- Must maintain current Michigan licensure as a Registered Nurse.
- Hospice certification preferred.
- A working knowledge of current pain and symptom management, and experience appropriate to the needs of hospice patients and their families is preferred.
PHYSICAL DEMANDS:
- Physical and health status which would accommodate delivery of duties and access to patient homes.
WORKING CONDITIONS:
- Works in a clean, well-lighted office environment, on nursing units and patient homes.
- Occasionally subjected to irregular working hours.
- Shares on-call responsibilities.
- May be exposed to communicable diseases and environmental hazards on the nursing unit and in the homes of patients.
- Occasional pressure due to multiple calls and inquires.
- May travel to homes during variable weather conditions.
- Must have reliable transportation and a valid driver's license.
