Position Summary:

The MSW Case Manager is responsible for facilitating coordination of multiple disciplines and resources for neonate, pediatric, adult, and geriatric patients to insure the highest quality care with the best outcomes for the patient in the most economical and cost effective manner. Collaborates with multiple disciplines and resources to facilitate transition of the patient to the appropriate site on the continuum of care. Integrates decisions of committees into the operation and activities of the department. Incorporates services to provide optimal health.

Minimum Education:
Graduation from an accredited Master’s Degree Social Work Program.

 

Minimum Work Experience:
Equivalent of one year of social work/health care experience.
 
Required Licenses/Certifications:
Current LLMSW and actively pursuing LMSW or LMSW
 
Required Skills, Knowledge, and Abilities:
Experience in psychosocial and therapeutic counseling
Recent clinical experience with specific patient population
Knowledge of care of neonate, pediatric, adolescent, adult, and geriatric patients
Ability to function successfully in an extremely challenging environment
Demonstrates leadership ability, communications, and interpersonal skills necessary to interact effectively with physicians, management, staff, external agencies/customers and patients/families
 
Preferred Qualifications:
LMSW
MSW Case Management Certification
Emergency Services Case Management experience
Hospital Social Work experience
Working knowledge/experience in utilization management, managed care, and payor issues
Discharge Planning experience
