General Summary:

  • Performs various basic care activities necessary in caring for the needs and comforts of terminally ill patients in their homes and under the direct supervision of the nurse.
  • Coordinates with the Interdisciplinary Team to provide patient/family care. Maintains continuity of care by ensuring patient needs are met.

EDUCATION AND EXPERIENCE:

 

Education:

  • High school diploma or equivalent is recommended but not required.
  • Successful completion of training and testing according to CMS regulatory requirements Part 42 CFR Section 484.36.

Certification:

  • None required.
  • However, Home Health Aides are encouraged to pursue registration with the State of Michigan Nurse Aide Registry as a Competency Evaluated Nurse Aide (CENA) if working in a long term care facility or hospice residence, or are certified as a Home Care Aide (HCA) through the Home Care University and/or Michigan Home Care Association. B. On-the-job.

 

Training:

  • A 4-8 week orientation period is recommended to familiarize the employee with policies and procedures as they relate to the hospice program.
  • Must successfully complete the National Homemaker-Home Health Aide Certification Program checklist for skills demonstration during orientation.
