Home Health Aide (full-time)- Hospice-Hospice Macomb, Clinton Township
General Summary:
- Performs various basic care activities necessary in caring for the needs and comforts of terminally ill patients in their homes and under the direct supervision of the nurse.
- Coordinates with the Interdisciplinary Team to provide patient/family care. Maintains continuity of care by ensuring patient needs are met.
EDUCATION AND EXPERIENCE:
Education:
- High school diploma or equivalent is recommended but not required.
- Successful completion of training and testing according to CMS regulatory requirements Part 42 CFR Section 484.36.
Certification:
- None required.
- However, Home Health Aides are encouraged to pursue registration with the State of Michigan Nurse Aide Registry as a Competency Evaluated Nurse Aide (CENA) if working in a long term care facility or hospice residence, or are certified as a Home Care Aide (HCA) through the Home Care University and/or Michigan Home Care Association. B. On-the-job.
Training:
- A 4-8 week orientation period is recommended to familiarize the employee with policies and procedures as they relate to the hospice program.
- Must successfully complete the National Homemaker-Home Health Aide Certification Program checklist for skills demonstration during orientation.
